7月26日,亚洲直接血管成形术大会(Asia Primary Angioplasty Congress,APAC 2025)在巴厘岛落下帷幕。大会期间,印度尼西亚青年医师Suryo Ardi Hutomo在主会场报告了他在厦门大学附属心血管病医院(简称“厦心医院”)参加“心苗”访问学者计划培训期间参与的1例高难度急性心肌梗死(AMI)救治病例,从众多参会病例中脱颖而出,荣获“最具挑战性AMI病例奖”(The Most Challenging AMI Cases Awards)第3名。
复杂危重病例的“中国方案”
Suryo医师报告的病例是一名78岁男性患者,因亚急性下壁心肌梗死、心力衰竭,合并重度二尖瓣反流、糖尿病及肾功能不全入院;实施再灌注治疗后进展为难治性心肌梗死-心源性休克(MI-CS),需紧急植入静脉-动脉体外膜肺氧合(VA-ECMO);然而VA-ECMO支持后出现左心室扩张综合征,血流动力学岌岌可危。在王焱院长的指导下,Suryo与厦心团队采用球囊房间隔造口术(BAS)联合国产Synflow®3.0微轴流泵的综合减压策略,迅速有效改善患者临床状态及血流动力学。
此类病例通常预后极差,死亡率高,VA-ECMO介入后又易因左心室负荷过重而引发灾难性并发症。该病例首次报道了国产微型轴流泵在VA-ECMO支持的 MI-CS中用于左室减压,验证了该器械与BAS联合策略的安全性和有效性,充分彰显了中国专家智慧与中国创新器械的协同力量,同时也生动诠释了心苗计划这一国际心血管专科人才培训项目的卓越成效。
印尼青年医生的“破局选择”
Suryo所在苏托莫综合学术医院(Dr. Soetomo General Academic Hospital)是印尼最大的公立医院之一。然而,当地的心血管医疗资源相对匮乏且分布不平衡,心血管介入治疗创新技术领域仍处于起步阶段。
因此,Suryo主动申请“心苗”访问学者计划项目,并于2024年2月赴厦心医院接受为期1年的冠脉介入专科培训。对于在厦心接受培训的机会,Suryo格外珍惜,“这里不仅医疗设备一流,更有大量复杂、高难度病例,是我在印尼难以接触到的。”
作为合作导师,李穗吉主任对Suryo赞赏有加:“他勤奋、严谨,对每一例复杂病变都记录详尽,善于主动思考、提问,并在团队指导下逐步掌握操作技能。”
结业归国仅三个月后,Suryo 便顺利取得印尼 PCI 及其他心血管介入手术资质。他由衷地说:“感谢厦心提供的平台,没有这一年高强度的实战训练,我不可能如此快速地通过考试并拿到执照。”
国际医疗合作的“厦心模式”
作为由国家国际发展合作署在联合国总部正式发布,并纳入国合署全球发展项目库的国际医学人才培养项目,心苗计划自2023年启动以来,已吸引来自12个国家的26名青年医师来华培训,搭建了中外医学交流与合作平台,推动全球医学人才的共享与发展。
与此同时,厦心医院积极前往学员所在国家医院开展手术指导,完成医疗技术帮扶的“双向奔赴”。2024年8月,厦心医院王焱院长团队曾受邀访问Suryo所在的苏托莫综合学术医院,与当地专家进行深入交流并签订合作协议,随后运用国产创新医疗器械成功指导实施两项当地首例创新介入手术,把“中国方案”转化为“当地实践”。(相关报道)

Heart Sapling Blossoms | Indonesian Trainee Wins “Most Challenging Case” Award at Leading Asian Medical Congress with Complex Case from XMCH
On July 26, the Asia Primary Angioplasty Congress (APAC 2025) concluded in Bali. During the event, Indonesian young physician Dr. Suryo Ardi Hutomo presented a complex acute myocardial infarction (AMI) case he participatedin during his training at Xiamen Cardiovascular Hospital Xiamen University (XMCH), under the Heart Sapling Visiting Scholar Program. His case stood out among many others and won 3rd place in the "Most Challenging AMI Cases Awards."
A "Chinese Solution"to a Critical and Complex Case
The case presented by Dr. Suryo involved a 78-year-old male patient admitted with subacute inferior myocardial infarction, heart failure, severe mitral regurgitation, diabetes, and renal dysfunction. After reperfusion therapy, the patient progressed to refractory myocardial infarction-induced cardiogenic shock (MI-CS) and required emergency veno-arterial extracorporeal membrane oxygenation (VA-ECMO). However, post-ECMO support, the patient developed left ventricular distention syndrome, leading to hemodynamic instability.
Under the guidance of President Dr. Wang Yan, Dr. Suryo and the XMCH team implemented a combined left heart decompression strategy using balloon atrial septostomy (BAS) and the domestically developed Synflow® 3.0 micro-axial pump, which rapidly and effectively improved the patient's clinical status and hemodynamics.
Such cases generally have a poor prognosis and high mortality rate. When VA-ECMO is applied, it may worsen left ventricular overload and lead to disastrous complications. This case was the first to report the use of a domestic micro-axial pump for left ventricular decompression in MI-CS under ECMO support. It verified the safety and effectiveness of combining the device with BAS, highlighting the synergy of Chinese clinical expertise and domestic medical innovation. It also vividly demonstrated the impact of the Heart Sapling Program as an international training platform for cardiovascular specialists.
An Indonesian Young Doctor's Bold Step Forward
Dr. Suryo works at Dr. Soetomo General Academic Hospital, one of Indonesia’s largest public hospitals. However, cardiovascular medical resources in the country are still relatively scarce and unevenly distributed. Interventional cardiovascular technologies are still in the early stages of development.
Motivated to improve his clinical skills, Dr. Suryo proactively applied for the "Heart Sapling" Visiting Scholar Program and began a one-year coronary intervention fellowship at XMCH in February 2024. He deeply valued this opportunity:"Here, not only is the equipment top-notch, but I also had access to numerous complex and high-risk cases—something I could hardly experience back home."
His mentor, Dr. Li Suiji, praised him:"He is diligent and meticulous, documenting each complex case in detail. He's inquisitive, proactive in asking questions, and gradually mastered interventional techniques under team guidance."Just three months after graduating and returning to Indonesia, Dr. Suryo successfully obtained PCI and other interventional surgery qualifications. He said sincerely:"I'm grateful for the platform XMCH provided. Without that intense year of practical training, I wouldn’t have passed the exams and obtained my license so quickly."
"XMCH Model" for International Medical Collaboration
The Heart Sapling Visiting Scholar Program was launched in 2023 and officially introduced by the China International Development Cooperation Agency (CIDCA) at the United Nations Headquarters. It has been included in the Global Development Project Repository. Since its inception, the program has attracted 26 young physicians from 12 countries, building a global platform for medical exchange and talent development.
Meanwhile, XMCH actively sends teams abroad to conduct surgical guidance in the trainees’ home countries, achieving reciprocal international cooperation. In August 2024, President Dr. Wang Yan and his team were invited to Dr. Soetomo General Academic Hospital, where they held in-depth exchanges, signed a cooperation agreement, and successfully guided two first-ever interventional surgeries using Chinese innovative medical devices. This transformed a "Chinese solution" into "local practice."
By combining "bringing in" and "going out", XMCH is implementing the Global Development Initiative with concrete actions and contributing Chinese wisdom to global health. President Dr. Wang stated:"The Heart Sapling Program not only imparts technical skills but also cultivates clinical thinking and hands-on ability. We aim to share China’s cardiovascular innovations and mature experience, enabling advanced therapies to take root locally and benefit more patients."
