病例

类风湿性关节炎(RA)和脊柱关节炎 (SPA)并存尽管非常罕见，但仍有可能二个疾病共存。

发生RA和SPA的并存的概率为1：50000~1：200000。

一例34岁男性，对称性多发性关节炎，炎性背痛，还有指炎。

化验：抗环瓜氨酸多肽抗体检测阳性，HLA-B27阴性。

腕关节磁共振成像显示：腕关节骨糜烂。

骶髂关节核磁显示：双侧骶髂关节活动性炎症伴有侵蚀性病变。

诊断：血清阳性RA和中轴SPA合并诊断

治疗：给予甲氨蝶呤和阿达木单抗，治疗临床效果良好。
